Why a patch is not proof of remediation

Merging a fix proves code changed. It does not prove the original risk is gone, that tests covered the failure mode, or that authority and scope were correct.

Require: impact and ownership, approved remediation, required gates, independent re-test of the original condition, and an evidence artifact with timestamps and evaluator context where available.
